HOW TO APPLY

If you meet the eligibility criteria then the application process is simple indeed. There are no forms to fill out. You just need to tell us:
  1. Your name;
  2. Your age;
  3. Your relationship to the Vietnam Veteran and his/her name;
  4. Your mailing address, contact telephone number and e-mail address (if you have one);
  5. What you need assistance for;
  6. Total cost and how you are going to meet any shortfall;
  7. When you need the money by; and
  8. A short (one paragraph) pen picture of yourself - what you have achieved to date and an indication of your life goals.
